The Ph.Lo fault is an indication of an input phase loss or large supply imbalance.
Check phase to phase, phase to ground and do the same on the output side to see if one or more phases are dipping right before the drive trips compared to the other two legs. Also, measure the DC Bus Link (between: DC+ and DC- when meter is set for an AC mode) to confirm if there is a significant ripple input/output voltage imbalance.
